The Role:


An exciting opportunity currently exists for a motivated, enthusiastic Grade 2 Social Worker in the role of Post-Acute Care Coordinator, providing coordinated, timely and seamless transition for patients and their families from inpatient care to home. 


The Post-Acute Care Program at RCH is a stated-wide service coordinated by the Complex Care Hub with care and support provided by RCH staff members and/or external organisations. Post-Acute Care provides short-term support at home to children whose health care needs have increased following an acute hospital admission or Emergency department presentation. The program aims to help children recover at home and prevent readmission to hospital. Services offered as a part of this program include; nursing care, occupational therapy, physiotherapy, personal care and respite.


The Post-Acute Care Coordinator works in partnership with medical, nursing and allied health staff and the Post-Acute Care team. Responsible for the co-ordination and oversight of the Post-Acute Care program this role provides a vital connection between the acute, sub-acute and community service systems.


The successful applicant will have proven experience formulating psychosocial assessments preferably within a healthcare context. As a primary resource to the team in supporting the psychosocial needs of patients and their families, the role requires a capacity to demonstrate initiative and leadership within a fast paced environment. The successful applicant will have demonstrated experience working with stakeholders at various levels within hospital and community settings.


All applicants must be eligible for membership of the Australian Association of Social Workers and meet AASW CPE accreditation standards. Experience in working within a health based setting is preferred.
