This Role: provides specialist care to infants, children and their families across both the Post-acute Care and Complex Care programs. Post-acute care provides short term support in the community to infants and children who have ongoing care needs following an acute hospital admission. The Complex Care program is a state wide multidisciplinary service that supports children with chronic and complex medical needs in their home environment.

 

To be successful in this role you will have:

  • Experience in providing assessment and goal directed intervention for patients with complex medical conditions and their families, including referral to community services to address ongoing functional needs 
  • The ability to negotiate with multiple stakeholders in a professional manner 
  • Commitment to evidence based practice, and integration into every day clinical care 
  • Demonstrated ability to work autonomously and within an interdisciplinary team
