Project Administration Officer – Transition Support Service
 

  • Part Time Fixed Term – 0.1 (7.6 hours) FTE commencing February 2019 for 12 months (with possibility of extension)

The Role:

The Transition Support Service provides integral support, preparation and care coordination for adolescents from the age of 15

and their parents/carers as they transition to adult and community services, and works in partnership with all clinical and

surgical specialties across the RCH to develop optimal transition pathways and processes which enable best long term care outcomes.

 

The Project Administration Officer will work under the direction of the Transition Manager / Principal Investigator (PI) and alongside the study Project Coordinator, to assist with participant recruitment and follow up in the International Interdisciplinary Transition Study.

 

This important 3-year study will evaluate the transition outcomes of adolescents and young adults (AYA) moving from the RCH to Victorian adult health services, and for AYA patients moving from Helsinki Children’s Hospital (HCH) to Helsinki University Hospital (HUH). Outcomes from this study will help to inform transition care globally.
