The Complex Care Hub team is a new interdisciplinary service that has been established to lead and coordinate the health care experiences for complex patients and their families. 

 

We are currently looking for an experienced Registered Nurse to join the Complex Care Hub. The RN role will be accountable for clinical practice and the delivery of care that is safe, evidenced based, comprehensive and patient family focused. This RN will work across all Complex Care Hub programs, including Post-acute Care, In-home healthcare Support, Schoolcare Program and Homecare Program.

 

This is an ideal role for someone with a strong paediatric skill base who has a passion for community nursing.

 

To be successful in this role you will have:

  • Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (AHPRA)
  • Recent experience in paediatric nursing and working with the community.
  • Evidence of on-going professional development as reflected in Professional Practice Portfolio.
  • A current,  full drivers licence for the State of Victoria which is appropriate for the type of vehicle being driven, and comply with any restrictions on their licence (e.g. wearing glasses) while undertaking hospital duties.
